Founded in 2023, by Kate Lewis, Connecting You Support Coordination is dedicated to making your NDIS journey less complicated.
Kate has been working as a Support Coordinator for over 5 years and has a wealth of knowledge and connections in the industry.
Kate prides herself on helping her clients feel less overwhelmed with 'everything NDIS' by providing a comfortable, prompt and efficient service.

We understand that everyone is different, so it is vital that each individual is supported in a way that benefits them.
For example, a client may not like speaking over the phone, so we ensure to text or email this client instead!

Going through life with disabilities is not easy. Today’s world expects us all to be strong and capable and is built assuming such. Support services help you maintain your independence by helping you navigate daily life tasks and engage with the community.
Your Support Coordinator can help you find suitable support providers to help you reach your goals!


Manage
A Support Coordinator should build a strong rapport with their client, to ensure they know what their goals are so that the budget can be managed accordingly. Support coordination is allocated in a plan to help a participant build their capacity navigating the NDIS system. Connecting You support coordination prides themselves on being approachable, prompt and knowledgeable so that you can feel comfortable in getting all of your questions and concerns answered!
Reflection
A Support Coordinator should have a sound understanding of their client's wants and needs and should educate them on what is reasonable and necessary under NDIS guidelines.
In the event that something does not work well for a participant, a support coordinator should have the knowledge and resources available to prepare reports and collate sufficient evidence to support a plan review request.

FREQUENTLY ASKED QUESTIONS
HOW DO I APPLY FOR NDIS FUNDING?
NDIS is extremely evidence-based, therefore, the more reports you can provide, the better!
Your reports should be from a specialist to give more grounding.
Keep in mind that NDIS will only support a lifelong disability, so your report/s should include how your disability affects your life now and in the future!
Once you've gathered your reports, the NDIS application form* will need to be completed;Â your GP or specialist will need to be involved in completing this form!

DOES A SUPPORT COORDINATOR ASSIST IN THE APPLICATION PROCESS?
A support coordinator generally doesn't become involved with a participant until their NDIS plan is approved. However, a support coordinator can be present at your initial planning meeting to support the participant in understanding NDIS terms, expressing their support needs, and providing moral support!

Connecting You is based in Hamilton, Vic and is happy to travel to surrounding regions such as Warrnambool, Portland, and Ballarat.
Connecting You has connections across the majority of Australia, so is happy to support participants remotely.
I DON'T HAVE SUPPORT COORDINATION FUNDING IN MY PLAN, CAN CONNECTING YOU STILL HELP ME?
Definitely! You can request for Support Coordination funding to be added to your plan by completing a Change of Situation form*. Connecting You can help you to complete this letter.
